I have had two adult only holidays in the carribean, and loved both of them. We went with our friends, so there were 4 adults on the holiday.
The 1st one was in Barbados, and the resort was split on two sites - one for anyone, and one for adults only. The deal was all inclusive, so we could use their restaurants, pools and mini golf course, but they could not use ours, if they had kids with them! Brilliant!
Two years later we went to Blue Bay Resort in the Dominican Republic, and again it was an all inclusive, adults only resort. Fantastic.
If you just want a break from your kids or grandkids, or just want adult only company, I strongly recommend the whole concept.
